Development of the Prototype

  • Steps in filling- up the form

 

Section 1. This section serves as the homepage of the Online Enrollment System. Enrollees are required to enter the required fields such as email address, school year, and question about the existence of LRN.

Section 2. This section contains student’s information such as PSA Birth Certificate No., LRN No., applicant’s name, sex, Birthdate, contact no., mother tongue and a closed-ended question about Ethnicity (Belonging to any Indigenous People).

 

 

Section 3. This section contains the student’s address.

Section 4. This section contains parent/guardian’s information such as name, age occupation, and cellphone no. for emergency. This also includes the question regarding the type of student of the enrollees.

 

Section 5. This section contains the questions that will be answered by returning learners (Balik-Aral) and those who shall transfer/move in only. The questions to be answered were last grade level completed, last school year completed, school name, school ID, and school address.

 

 

Section 6. This section must be filled by the enrollees. The questions in this section are semester, track, strand, and consent form with the signature of the parent/ guardian and date signed.

Section 7. This section is to be filled by the class adviser or DepED personnel only. This contains the date of first attendance and the button which may allow the enrollees to have the copy of their responses.
